Intake Form

Example

Babysits is a supply and demand platform. We facilitate parents in finding a babysitter and babysitters in finding a babysitting job. We do not screen the parents or babysitters that sign up on Babysits. This means that parents and babysitters are wholly responsible for making an accurate selection.

Please ask the babysitter to fill this form out before they start babysitting for you for the first time.
